Using our finely tuned data base, we will evaluate and recommend appropriate settings, close to the family when possible, and within a price range manageable.
Emotional Needs
Recognizing the importance of good mental health to one’s overall sense of well-being, is essential to developing a comprehensive plan.
As care managers we utilize standardized tests such as the Folstein Mini-Mental State Examination to assess our client’s emotional status.
Dementia, Alzheimer’s, Depression, Mood disorders, these are all examples of conditions revealed using such tests. Counseling, medications, or hospitalization may be needed to address these issues.

Social Needs
Stressful life events can increase susceptibility to illness. Elderly persons who have few social and community ties are at increased risk of disease or institutionalization.
Evaluation of one’s informal, formal and semiformal support systems will be instrumental in determining the needs of a senior. Facility placement must be evaluated for therapeutic and recreational offerings, to insure the ability to maintain an active, involved lifestyle.

Medical Needs
Activities of daily living and the ability to manage one’s own medication administration are two areas of care critical to the assessment of a loved one’s independence.
A values history to include discussion about Durable Powers of Attorney, Living Wills, Advance Directives and End-of-Life care must be attained.
Medical care should be reviewed by a licensed practitioner to keep a loved one safe and healthy. Ability to make and keep physician appointments, pay for needed medicines and prevent over-medication of an elder, must be evaluated.

Finances are always an issue when evaluating providers for Home Health, Durable Medical Equipment, Private Nursing, Counseling services, Assisted and Transitional Living arrangements, and other services required.
